By entering the Pets Best Insurance My Vet’s the Best Contest (“Contest”), you agree to abide by these Official Rules and any other terms and conditions referenced in these Official Rules.
ELIGIBILITY: This Contest is only open to legal residents residing in one (1) of the fifty (50) United States (including the District of Columbia) who are 18 years of age or older at the time of entry. Employees of Pets Best Insurance and the household and immediate family members (defined for the purposes of this Contest as a parent, spouse, child, and sibling) of such employees are not eligible to enter or win. All licensed veterinarians are eligible for nomination. Sweepstakes is subject to all federal, state and local laws and regulations. Void where prohibited or restricted by law.
ENTRY PERIOD: This Promotion begins at 12:01 a.m. Mountain Time (“MST”) on January 11, 2011, and ends at 5:00 p.m. MST on December 31, 2011 (“Promotion Period”). By participating in the Contest, each entrant unconditionally accepts and agrees to comply with and abide by these “Official Rules” and the decisions of the Sponsor, including the interpretation of these Official Rules and its exercise of discretion, which will be final and binding in all respects. There is (1) one method of entry for this Contest.
HOW TO ENTER: To enter the Contest, email a story, pictures and/or video telling us why your pet’s veterinarian or veterinary hospital is the best. All entries must be submitted to petsbestpromos@gmail.com. Include your pet’s name, your full name and address, and the full name and address of your veterinarian. Stories will be shared on the Pets Best Insurance website (www.petsbest.com) and Facebook Page at the discretion of the Pets Best Insurance Marketing Team. Limit one (1) entry per pet.
Information collected from entrant via the entry form will only be used to notify the winner. All entries become the property of Pets Best Insurance and will not be returned.
WINNER SELECTION: Once per calendar year quarter, a winning story will be chosen on dates to be determined by the Pets Best Insurance Marketing Team.
PRIZE: The pet owner who submitted the winning story will receive a $100 gift card. The veterinarian or veterinary hospital nominated in the winning story will receive $500 to be used toward the veterinary treatment of a stray animal/s, or toward the treatment of animal/s whose owner cannot afford to pay for necessary veterinary care (“Rainy Day” Fund). The winning veterinarian/veterinary hospital will be requested to report how the prize money was used; reports may be used in press releases or other marketing materials.
No substitution, transfer, or assignment of prize permitted, except that Sponsor reserves the right to substitute a prize of equal or greater value in the event an offered prize is unavailable. Taxes on the prizes are solely the responsibility of the winner.
WINNER: The winning pet owner will be notified and confirmed via email, in response to the email address used to submit winning story. If notified winner is not eligible, forfeits prize or declines prize, there will be no alternate winner. Winning veterinarians/veterinary hospitals will be contacted via telephone.
SPONSOR AND ADMINISTRATOR: This Contest is sponsored and administered by Pets Best Insurance, 2710 Sunrise Rim Road, Boise, ID 83705. This Contest is in no way sponsored, endorsed or administered by, or associated with, Facebook. You understand that you are providing your information to Pets Best Insurance and not to Facebook. The information you provide will only be used for Contest winner notification.
